PROVIDER SERVICES — Login Information Request

You will need your own unique login information (user name and password) any time you want to update your provider information for Aging Projects, Inc. We suggest that you keep these in a safe place that’s accessible when needed. To get your login information:

• Email the webmaster. You will get a response within 24 hours—usually sooner.

Note that once you have your user name and password, you can UPDATE your provider information to change your user name and/or password to something more easily remembered.
