Provider’s Statement of Service(s):
Caregiver services are available for those who are caring for an older adult with dementia or other physical issues. Counseling support is available to guide you in your journey.
Funding is available for respite care services (in home care, adult day care, or overnight stays) and for supplemental services to assist with other challenges in and around the home. The care recipient must be at least 60 years of age or be under the age of 60 and have Alzheimer’s disease or a related dementia and the care recipient must require assistance with at least 2 activities of daily living or have significant memory loss.
