Provider’s Statement of Service(s):
CarePartners PACE (Program of All-inclusive Care or the Elderly) is a program of people age 55 or older that need support to remain safely at home. It provides community-based care and services for people who might otherwise need nursing home level of care. A PACE program works to promote and sustain the independence of seniors, and help those who want to live at home, stay at home.

Services include, but are not limited to: Primary Care, Prescription Drugs, Home Care, Physical Therapy, Occupational Therapy, Adult Day Care, Recreational Therapy, Meals, Dentistry, Nutritional Counseling, Social Services, Laboratory/X-ray Services, Social Work Counseling and Medical Transportation.

Provider is not a registered non-profit.
